Application Assistance

We help you fill out your applications.

The application process can be complex, overwhelming, and very time consuming. Evans Disability is here to help!

We try to make the process of applying as simple as possible. We sit down with our clients and help them fill out the application. We help our clients fill out the application online, on paper and even over the phone with SSA. We will make sure that you understand the questions you are being asked before you answer them. We also understand how difficult it is for people who are disabled to travel. We can help people fill out their applications over the phone, video chat or even e-mail with them.

When you complete an application for Social Security disability benefits, you are submitting information to the federal government and swearing under the penalty of perjury that the information you provide is true. That is a key part of why it is helpful to have Evans Disability represent you from the start.

Many firms tell clients to fill out their applications and call back once they are denied. Evans Disability, however, likes to work together with our clients from day one. We review everything before it is submitted to Social Security. Our goal is to put you in the best position to win your claim.

These questionnaires are very helpful in a disability claim. We win many cases at the initial level because we work to get this information in your file from day one. We also sit down with you and provide a list of things you can do to help your claim succeed.

Once an application is submitted, Social Security will begin to send the claimant a high volume of additional forms to complete. When claimants apply on their own, they are often overwhelmed and distressed by the amount of information Social Security requests. With Evans Disability, we continue to help our clients through the whole claims process. We help our clients fill out the numerous forms that Social Security sends throughout the process. You will find that we are a valuable asset to have when it comes to assisting you in processing and filling out the required paperwork.

We believe it is very important that you get advice when filling out these documents, since how you fill them out can have a major impact on your claim.

When you file an application you will have to provide information about your education, medical conditions, treatment for those conditions and work history for the past 15 years. Social Security uses the following forms to capture that information:

(Forms to start your claim):

SSA-16-BK, Application for Disability Insurance Benefits (for SSDI only)

SSA-8000-BK, Application for Supplemental Security Income (for SSI only)

SSA-3368-BK, Adult Disability Report

SSA-827-BK, Authorization to Disclose Information to the Social Security Administration

Another important document, for those who are seeking representation with their application, is the Form SSA-1696

SSA will also ask you to fill out the following forms after you have applied:

SSA-3373-BK, Function Report (Adult)

SSA-3369-BK, Work History Report

Evans Disability helps with these forms as well.

To help this process go as smoothly and quickly as possible the following information should be ready and at your fingertips:

Citizenship, birth and residency

Personal information: children, marriage, divorce, etc.

Work history for 15 years prior to your disability

Financial information: life insurance, bank accounts, stocks, 401Ks, etc.
